Description
Starting pay: $20.50 per hour
- Assists in assessing the social service needs and problems of the patient and assists in preparing the social service plan based on assessment incorporating the physiological and developmental age of patients.
- Assists in providing supportive counseling to the patient and family or significant other, with appropriate follow-up visit or phone call to ensure that goals are met.
- Refers to appropriate community resources for information and services.
- Participates in interdisciplinary team meetings and/or individualized conferences.
- Evaluates insurance/financial resources and assists patient/family to obtain whatever services for which they qualify.
- Assists patient/family with Nursing Home placement, when needed.
- Participates in the planning, development and presentation of staff inservice education programs.
- Serves as consultant to departmental staff of individual problems encountered with patients.
- Interprets the availability of community resources to departmental staff.
- Documentation and telephone are completed and returned to chart within two workdays.
- Incorporates the physiological and developmental age of patients into the overall plan for the delivery of patient care.
- Participates in interdisciplinary team meetings and/or individualized conferences.
- Assists Social Services Staff with email, faxes and other clerical tasks.
- Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
- Participates in all required in service education programs and meetings.
- Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Performs other duties as assigned.
Position Requirements
Required Skills/Abilities:
- Knowledge of regulations surrounding compliance with Medicare Conditions of Participation.
- Ability to function as a resource to patients and families during times of high stress and volatility.
- Ability to interact with a mixed patient population
- Excellent interpersonal and communication skills, including providing support for patients and families, resource providers, members of the clinical team and employees.
- Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing.
- Well organized to manage patient and family needs, provide resources and complete timely documentation
- Ability to work both independently with sound decision-making, and as a part of an interdisciplinary team.
- Strong assessment and documentation skills.
- Proficiency in operating a computer; including a working knowledge of Microsoft applications (Word, Excel, and Outlook) and an electronic medical record.
Education and Experience:
Required Education: Bachelor’s Degree in Social Work.
Preferred Education:
Required Experience: One year of Social Work or counseling experience.
Preferred Experience: Prior experience in a healthcare setting.
